Insurance Information
The physicians at Hamilton Medical Group participate in many health insurance plans. In order to ensure that you receive the maximum benefits of your insurance plan, we request that you verify that the physician you are scheduling an appointment with is on your plan at the time you schedule your appointment. In addition, we request that you bring all of your insurance cards with you on each visit to our clinic to ensure the accuracy of your insurance information and to expedite the check-in process.

Appointments
Please call the office to make an appointment in advance.
If you are unable to keep your appointment, please call us as far in advance
as possible so we may use that time to see another patient in need of care.
We make a sincere effort to adhere to our appointment schedule and appreciate
your patience if we are late due to emergencies or hospital surgery.

Provided below are various forms that require completion prior to your visit and/or surgery. Please download the appropriate form as directed by your physician and/or staff by clicking on the name of the form. If you can not access the form, please click on the Adobe box. This will allow you to download this software, providing access to the forms. Please bring the completed form/s with you to your scheduled visit. This will help expedite the registration process. Fees & Payments
It is the policy of Hamilton Medical Group to collect any payment due from the patient (such as copayment, coinsurance, deductible, etc.) when you arrive and check-in for your appointment. Hamilton Medical Group will make every effort to verify your coverage prior to your scheduled appointment, however, we cannot guarantee coverage of services by your insurance company. All locations accept Visa, MasterCard and Discover as an option of payment.

Prescriptions & Renewals
Please request all prescriptions and authorizations for renewals
between 8:30am - 5:00pm, Monday to Friday, when our full records are available.
Renewals requested at other times will be filled only for extreme or emergent
circumstances.
